In an alternate version of 1949 Japan in which World War II never happened, the Japanese capital of Teito is home to both an ultra rich upper class and the dirt poor masses. The city is thrown into a state of panic when a phantom thief called “The Kaijin (Fiend) with 20 Faces” (K-20 for short) begins to use his mysterious abilities to steal from the rich and give to the poor. One day a circus acrobat named Heikichi Endo (Takeshi Kaneshiro) is framed for K-20’s crimes and becomes determined to clear his name. He teams up with K-20’s next target, a wealthy duchess named Yoko Hashiba (Takako Matsu) and her detective fiancé (Toru Nakamura), to take K-20 down once and for all.

While Rena and Maki recover from their injuries, the other SORD members jet off overseas for a school trip. Within moments of their arrival, though, the Mihama gang are pulled into a manhunt for a SORD deserter, assisted by student Sylvia and Velvet of St. Aile's International School. The film's plot follows that of the game closely. Nariko, a member of an ancient tribe who have guarded an ancient magical sword for thousands of years, is charged with bringing the sword to its destined wielder, a deity reincarnated who will bring peace to the land. But on her quest, Nariko must wield the sword herself against the forces of King Bohan, who wishes to possess the weapon himself.

A film adaptation of the Holy Emperor story arc, which primarily depicted the conflict between Kenshiro and Souther. New characters Reina, one of Raoh's army officers who falls in love with him, and her brother Soga, Raoh's advisor, play an important part with much of the plot involving Raoh's relationship with Reina as he conquers the land; most of this portion is new content exclusive to this film. The other side of the story is the retelling of Ken's attempt to save and protect the villagers from Souther's army with the help of Shū. There is also a small subplot of Bart returning to his home.
