A down and out cop lands the case of a lifetime when four suspects are nabbed in the assassination attempt of a prime time journalist. The case turns out to be a devious maze where nothing is what it looks like. The pursuit of it leads him to the dark netherworld - the 'Paatal Lok', and to shocking discoveries in the past of the four suspects.

The movie follows the journey of Ranveer Malhotra, who against all the odds, has to face deadly obstacles on a plane in order to survive.

Inside Cama Hospital, one of the sites targeted during the 2008 Mumbai terror attacks, nurses, ward boys, cleaners, lift operators, security personnel, and administrative workers keep 400 people alive while armed assailants struck the city around them.
