The Americans are swiftly closing on Okinawa, an island just south of the Japanese mainland. The Imperial command sends top generals and several army divisions to defend it at all costs. The mission quickly degenerates as vital resources and troops are diverted to other islands. After a civilian evacuation ends in tragedy most of non-combatants are forced to remain on the island. Many convert to soldier status. Tokyo sends mixed messages that squander time and resources, as when they order the defenders to build an airstrip for aircraft that never come. The truth soon becomes obvious: the high command decides that the island cannot be held and effectively abandons the Okinawan defenders. When the Americans land many troops are deployed in the wrong places. As the slaughter mounts, a suicidal attitude takes hold. Okinawa becomes a death trap, for civilian volunteers and non-combatants as well.

South of Tokyo is a small island until quite recently relatively unchanged since feudal times. Now, however, it is being made into a tourist's playland and the girls, who had been abalone divers, now become geisha, and the old ways of the island are all topsy-turvy. One of the girls, more sensitive than the rest, sees that here, too, people are motivated only by greed and decides to leave and try to find a better life elsewhere.
Two soon-to-be kamikaze pilots stop by a local school near their base to play the piano one last time, leaving a deep impression on a teacher. Years later, she seeks out the relatives of the pilots when the piano is old and about to be discarded.
The story is set in Sumidagawa Hospital, an emergency hospital in downtown Tokyo. The head of surgery there, Iwashita Ken, is a brain surgeon who has just returned from the United States . The story depicts the various facets of the hospital, including the interactions between doctors and patients, the power struggle within the hospital, and Asou Yukie, a recent nursing school graduate who was assigned to the hospital unaware of the complex internal affairs that exist.
